Does Cataract Surgery Hurt During the Procedure?

Short answer: No, cataract surgery is not painful for most people during the procedure.

Here’s what actually happens:

Before we even start, we give you numbing drops. Yes, they sting for about 10-20 seconds — think of it like when soap gets in your eye. That’s the worst part for many patients, honestly.

Then we add local anaesthesia around the eye. By the time we begin the surgery, your eye is completely numb.

So What DO You Feel?

Most of my patients describe it as:

  • Pressure — like someone gently pressing on your eye
  • Bright lights — very bright, sometimes colourful swirls (that’s normal!)
  • Weird sensations — not pain, just… strange

One patient told me, “Doctor, it wasn’t painful at all. Just weird. Like I knew something was happening, but it didn’t hurt.”

That’s exactly right.

Is It Normal to Feel ANYTHING During Surgery?

Yes! Feeling pressure is completely normal. Feeling like someone is touching your eye (because, well, we are) — also normal.

What’s NOT normal: sharp, stabbing pain.

If you feel actual pain during the procedure, not just pressure or discomfort, but real pain, you need to tell your surgeon immediately. We can add more anaesthetic. Don’t suffer in silence, thinking “this is how it’s supposed to be.”

I tell every patient before surgery: “If something hurts, tell me right away. You shouldn’t be in pain.”

But Doctor, I Read on Reddit That Someone Had TERRIBLE Pain…

Look, I’m not going to lie to you. I’ve read those threads too. And yes, a small number of people do report pain during or after cataract surgery.

Here’s what’s usually happening in those cases:

During surgery:

  • The anaesthesia wore off too quickly (rare, but it happens)
  • Extra sensitivity due to anxiety or muscle tension
  • Previous eye trauma or inflammation, making the eye more sensitive

After surgery:

  • Some people are more sensitive to medicated eye drops (they can burn!).
  • Dry eye making everything feel scratchy.
  • Inflammation that needs additional medication

But here’s the thing — these are the exceptions, not the rule. Most people genuinely have a smooth, pain-free experience.

The problem? People who have normal, boring surgeries don’t rush to Reddit to post about it. The ones who had problems? They post. That’s why it can seem scarier than it actually is.

  • Your eye might feel scratchy or gritty (like you have an eyelash stuck)

This usually lasts 24-48 hours and gets better quickly.

When to Call Me Immediately

Now, if you have severe pain after surgery, pain that’s getting worse, not better, call the clinic right away. That could mean:

  • Infection (very rare, but we watch for it)
  • Increased eye pressure
  • Inflammation that needs treatment

Don’t wait. Don’t Google. Just call.

“Doctor, Why Am I So Scared Even Though You’re Saying It Doesn’t Hurt?”

I completely understand. The mental aspect of knowing someone is working on your eye while you’re conscious, that’s what freaks people out. Not the actual pain.

During surgery:

  • We give you mild sedation to help you relax (you’re awake but calm)
  • I’ll talk you through what’s happening
